THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY. THE PRIVACY OF YOUR MEDICAL INFORMATION IS IMPORTANT TO US.

OUR LEGAL DUTY
We are required by law to protect the privacy of your protected health information (“medical information”). We are also required to provide you this notice about our privacy practices, our legal duties, and your rights concerning your medical information. We must follow the privacy practices described in this notice while it is in effect.
Breach Notification: In accordance with Texas law, we will notify you following a breach of your unsecured protected health information within 60 days of the discovery of the breach.
USES AND DISCLOSURES OF YOUR MEDICAL INFORMATION
Treatment: We may disclose your medical information, without your prior approval, to another dentist or healthcare provider providing you treatment for the purpose of evaluating your health, diagnosing medical conditions, and providing treatment.
Payment: Your medical information may be used to seek payment from your insurance plan or from you.
Health Care Operations: We may use and disclose your medical information, without your prior approval, for health care operations. This includes quality assessment, improvement activities, and provider reviews.
Substance Use Disorder (SUD) Records: Per 42 CFR Part 2 (updated Feb 16, 2026), any records regarding SUD treatment are subject to heightened confidentiality. We will not disclose these records without your specific written consent or a court order. Recipients of such records are prohibited from re-disclosing them without your express authorization.
Family and Friends: We may disclose information to a family member or friend involved in your care unless you object.
Reminders: We may use medical information to send appointment reminders via US Mail, email, or telephone.
No Sale of Data: Texas Dental does not sell your personal or medical information. We do not use your data for targeted advertising without your prior express consent.
YOUR RIGHTS (TEXAS & FEDERAL)
Access & The 15-Day Rule: You have the right to inspect and get a copy of your health records. Pursuant to Texas HB 300, if you request an electronic copy of your records, we will provide them within 15 business days of your written request.
Texas Consumer Privacy Rights (TDPSA): As a Texas resident, you have the right to:
- Confirm if we are processing your personal data.
- Correct inaccuracies in your personal data.
- Delete personal data provided by or obtained about you.
- Obtain a copy of your data in a portable and usable format.
- Opt-out of targeted advertising or profiling.
Universal Opt-Out: We honor Global Privacy Control (GPC) signals or other recognized universal opt-out mechanisms transmitted by your browser.
Additional Rights: You have the right to request an Accounting of Disclosures, request Restrictions on who receives your info, and request Confidential Communications.
COMPLAINTS
If you believe your privacy rights have been violated, contact our Privacy Officer at the number above. You may also file a complaint with the Office for Civil Rights of the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
