We are also required to provide you this notice about our privacy practices, our legal duties, and your rights concerning your medical information. We must follow the privacy practices described in this notice while it is in effect.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Breach Notification:<\/strong> In accordance with Texas law, we will notify you following a breach of your unsecured protected health information within <strong>60 days<\/strong> of the discovery of the breach.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">USES AND DISCLOSURES OF YOUR MEDICAL INFORMATION<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Treatment:<\/strong> We may disclose your medical information, without your prior approval, to another dentist or healthcare provider providing you treatment for the purpose of evaluating your health, diagnosing medical conditions, and providing treatment.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Payment:<\/strong> Your medical information may be used to seek payment from your insurance plan or from you.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Health Care Operations:<\/strong> We may use and disclose your medical information, without your prior approval, for health care operations. This includes quality assessment, improvement activities, and provider reviews.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Substance Use Disorder (SUD) Records:<\/strong> Per <strong>42 CFR Part 2<\/strong> (updated Feb 16, 2026), any records regarding SUD treatment are subject to heightened confidentiality. We will not disclose these records without your specific written consent or a court order.
